Sand Castle Studios (SLurl) has produced NESIM, a cool nursing simulation in Second Life.
John Miller, an instructor using the simulation, blogged about it here.
The snapshot (left) shows the instructor’s Heads Up Display (HUD) on the left and the student’s HUD on the right.
Using the HUD the instructor can make various physical things happen to the virtual patient (such as a sudden increase in blood pressure).
The student uses the HUD to respond with interventions such oxygen, various drugs, defibrillators, and so forth.

Students can work alone or in teams.
Very cool.
